Army report cites systematic failures in 2025 helicopter and jet collision that killed 67
A U.S. Army investigative report, completed in May and seen by Reuters, concludes that a January 2025 collision between an American Airlines regional jet and an Army Black Hawk helicopter near Ronald Reagan Washington National Airport that killed 67 people resulted from multiple systemic failures. The report points to two broken radio transmissions that prevented the helicopter crew from receiving critical air-traffic-control information. It also cites technical issues with the Black Hawk and a pattern of crews not flying required semiannual minimum hours. Many helicopters in the battalion, including the one involved, lacked Traffic Alert and Collision Avoidance systems. The report also says Army-Federal Aviation Administration coordination in the Washington area was insufficient.
