A new study in the Journal of Perinatology examines the “physiologic signatures” seen during extubation readiness trials (ERTs) in neonates older than 35 weeks’ postmenstrual age. Led by G.P. Lima, A. Kummaragunta and J.G. Ortega, the research looks at what clinicians can observe when temporarily reducing ventilator support to see if an infant can sustain breathing without problems such as oxygen desaturation, carbon dioxide retention, apnea, or cardiovascular instability. The paper highlights why timing matters: prolonged mechanical ventilation can contribute to airway injury and infection, while early removal can require reintubation and add stress.
