A qualitative study from Tehran, published in Nursing Open, describes how thirst persists and dominates the experience of mechanically ventilated ICU patients even after extubation. Researchers at Tehran University of Medical Sciences interviewed 20 patients about five hours after breathing tubes were removed. All were mechanically ventilated, alert, had Richmond Agitation-Sedation Scale scores between -1 and +1, and showed no delirium on CAM-ICU. Using conventional content analysis, the team extracted 501 open codes and organized them into six subcategories under three overarching themes: Unrelieved Thirst, Seeking Relief, and Emotional Collapse and Withdrawal. Patients reported intrusive, repetitive thoughts about water, fear of dying from thirst, and trouble even sleeping, while some tried cognitive reframing by telling themselves the deprivation was temporary.
